- Based on the information gathered through the family health assessment, recommend age-appropriate screenings for each family member. Provide support and rationale for your suggestions.
- Choose a health model to assist in creating a plan of action. Describe the model selected. Discuss the reasons why this health model is the best choice for this family. Provide rationale for your reasoning.
- Using the model, outline the steps for a family-centered health promotion. Include strategies for communication.

FAMILY HEALTH ASSESSMENT
Family structure
The family described in this report is comprised of four members. The family is
headed by the father, who is an African American. The father, who is 52 years works as a
casual labourer in a local company. The family moved to the current location from California
where he had lived with their ancestors. The mother is a 42 years old African American. The
family, therefore, is made up of a standard background from a racial and ethnic perspective.
The mother is a secretary at one of the local companies. The family stands out as a middle
class in the community. The primary reason behind this conclusion comes from the
evaluation of the total income which the family members earn. The two children are a 15 year
old boy and 12 year old girl respectively. Also, the family observes a strictly religious
concept based on traditional values and beliefs. The father, in this case, acts as the ultimate
head of the family and forms the central point of cohesion and problem solving whenever an
issue arises. The family resides in a region mainly occupied by African Americans. The
reason for the selection of the region by the family is probably because of the desire to
achieve the feeling of self-identity.
Health behaviors
The family, according to the interview, is observant of the various health-related
concepts. The interview finds out, however, that the family members have not subscribed to
given health programs such as Medicare or Medicaid despite being compliant and qualified.
The reason behind the failure to subscribe to such programs is based on the concepts of
reduced income. Additionally, the family observes a reasonably balanced diet based on the
ideas that each meal must contain a few fruits. Significant meals are comprised mainly of
fruits and vegetables. The family is at a better health position except for the C-Diff and H-

Pylori conditions witnessed in recent months. Also, during the course of the interview the
family was found to not be suffering from obese conditions due to their nutritional intake.
Functional health pattern strengths
The first functional health pattern which the family exhibits and scores the best
includes sexuality and role (Donolly et al., 2018). The family, especially the father and the
mother, undertake their roles together. In this case, the father may undertake roles such as
cooking, while the mother undertakes the other tasks related to the male gender. On the other
hand, sexuality, in this case, acts as the second-best. In this case, the father and mother agree
that they are intimate and discuss their relationship matters together as a single unit. Also, it
is essential to note that the family is based and built on trust.
Health barriers
In this case, the interview outlined a few issues which have affected how the family
observes health practices. One of their weaknesses which the family exhibits are the inability
to observe a given health program (Oakley, 2016). The lack of a healthcare program such as
Medicare and Medicaid places the family at a disadvantage concerning services offered, such
as affordable care. The second barrier includes poor nutrition practices. The family reported
cases of C-Diff and H-Pylori infection in the past few months. These issues are brought about
by the existence of poor health and nutrition patterns. Three remaining issues arise in this
case including poor coping practices. For instance, the family members do not seek help from
the respective experts, such as healthcare practitioners or psychologists.

FAMILY HEALTH ASSESSMENT
Family system theories
This theory came as a result of Dr Murray suggests that a health practitioner may not
effectively understand a patient through the use of isolation from the other members of the
community (Kaakinen et al., 2015). The successful application of this theory ensures that the
healthcare provider can meet most of the health objectives through ensuring that there is the
active involvement of the affected patients to enable the practitioner to learn about the
challenges that they may suffer. In the presented case, the healthcare practitioner ensures that
they refrained from isolating the parents based on their background. Another application of
this theory that led to the successful accomplishment of health objectives is ensuring that they
provided them with all the requirements that they needed to ensure the success of the health
promotion of health in this community. Training the family about the issues that they would
suffer as a result of the failed application of effective health management strategies in
society. The health practitioner also ensured that the patient learned how they can improve
their overall health by providing that they use of effective health strategies.
